PM Patient Demographics Forms Template Window Is Blank

On Friday 9/13/2013 we released an update to our Practice Management application.  Since the release some of our clients have reported an issue where the Forms Tab in the Patient Demographics is blank.

There is a quick fix for this issue, if you clear your Temporary Internet Files and then log back into AdvancedMD, your forms will be listed.  Please note that this will have to be done on each PC that is experiencing this issue.  In some cases rebooting your PC is required after clearing the Temporary Internet Files.

To delete the files in the Temporary Internet Files folder, follow the steps for your version of Windows.

Windows 8

  1. Press the Windows Key + C (to bring up the Charms Fly Out)
  2. Click Settings, click Control Panel, and then click Internet Options.
  3. Click the General tab, and then click Delete under Browsing history.
  4. Click Delete all, click Yes to confirm that you want to delete this information, and then click OK.

Note: make sure to uncheck Preserve

Windows 7 and Windows Vista

  1. Click Start Start button, click Control Panel, click Network and Internet, and then click Internet Options.
  2. Click the General tab, and then click Delete under Browsing history.
  3. Click Delete all, click Yes to confirm that you want to delete this information, and then click OK.

Earlier versions of Windows

  1. Exit Internet Explorer.
  2. Exit any instances of Windows Explorer.
  3. Click Start, click  Control Panel, and then double-click Internet Options.
  4. On the General tab, click Delete Files under Temporary Internet Files.
  5. In the Delete Files dialog box, click to select the Delete all offline content check box.
  6. Click OK two times.

Real Time Transaction Processing issues – eEligibility/ Claims Processing

Update 9/24/2013 5:30PM :  Relay Health continues to work on a solution to the issue.   It is expected that this issue will continue in to tomorrow.  As more information becomes available we will notify you.

Eligibility should function during the evening and off peak times, but as the demand increases their ability to process transactions in real time will decrease.   During peak times 8:00 AM – 3:30 PM MST eligibility should not be used as it will generate a backlog of requests which will delay processing once the issue is resolved.

All Claims processing is complete as of this posting.  Depending on the volume of claims submitted, future claims processing may be delayed, until the issue is resolved.

Relay Health has notified us that they are continuing experiencing issues with Real Time transaction processing.  This Issue could impact the following areas:

• Eligibility (270/271)
• Claim Status
• ePrescribing
• Address/Credit Check

Relay Health is working to restore service as quickly as possible.

What is an MSP Code And Claim Exclusion: FK PSBR0500 MISSING INSURANCE TYPE CODE

MSP Stands for Medicare Secondary Payor.  MSP codes are used to explain why Medicare is the Secondary carrier on a claim claims.   Any Claim submitted with Medicare as the secondary will require an MSP code.

The MSP codes is added in the Patient Demographics section of AdvancedMD, and not at the claim level, because the reason a patient has medicare as the secondary doesn’t typically change.

How do I add an MSP code:

  1. From Patient Demographics, click the Insurance tab, highlight the Medicare secondary carrier
  2. Select the correct MSP Code | Save

How do I know which code to select?

The reason a patient has Medicare as a secondary is specific to the patients situation.  Consult the table at the end of this article or have the patient consult their Medicare representative for clarification.

Do I need to add an MSP code to the Primary carrier also?

No, The MSP Code should only be selected on Medicare when it is the secondary.

What if I select the wrong code.

Medicare requires a code that directly relates to the reason the patient has Medicare as the secondary.  If the code sent on the claim doesn’t match Medicares records the claim will be rejected.  FK:PSBR0502:INSURANCE TYPE CODE NOT ALLOWED is an example of an exclusion for the wrong MSP code.  To resolve this issue select the correct MSP code and resubmit the claim.

Exclusion: CH Missing Payer Claim Control Number

What is a Payer Claim Control Number/ICN Number?

A Payer Claim Control Number indicates the claim is a replacement or void to a previously adjudicated claim. The Payer Claim Control Number changed from ‘Situational’ to ‘Required’ in 5010.

When does this exclusion happen? 

This exclusion occurs when a claim is trying to be re-billed to a payer marked as corrected but without the original reference number.

How to tell if a claim is being marked as corrected.

This check box is found in the Charge Detail Screen.ICN1

 

 

How do I add the ICN number.

Go into the Extra Information tab in the Charge Detail Screen to find the Original Reference #.ICN2

 

 

 

Why is this option available?

Typically, a claim will be marked as corrected if the payer is requesting the claim be re-billed to them as a corrected claim. The Original Reference number is the ICN number on the EOB from the payer.

However, most claims do not need to be marked as a corrected claim if being sent to the payer a second (or even third or more) time.

Why most claims do not need this.

The AdvancedMD billing system auto generates a letter at the end of the visit number. Ex: 1234A, the A tells the payer that this is the first time the claim has been billed to them. If they send it back with a rejection, or the Clearinghouse sends the claim back with an exclusion, the second time the claim is billed out it will go out as 1234B. Each time the claim is rebilled, the letter changes to the next letter in the alphabet.

How is this exclusion fixed?

To fix this exclusion, determine if you are truly billing a corrected claim or not. If it is a corrected claim, find the ICN number off of the EOB and insert the ICN number into the Original Reference # field. If this claim does not need to go out as correct, simply un-check the Correct Claim box, and select re-bill.

*Note: Medicare does not accept corrected claims at all.

2014 Medicare Fee Schedule

The 2014 Medicare Fee Schedule is now Available.

You can update your current allowables by doing the following:

Access your fee schedules by going to Master Files|Fee Schedules.  Click on the allowable fee schedule you want to update from the table at the left.

Once you have your current allowable fee schedule chosen, click the ADD button.  This will open the Add New Fee Schedule Version window.  Change your Source Fee Schedule to the 2014 Medicare schedule, and check the increase all charges by box at the bottom of the window.  Input the percentage you would like to increase the Medicare Fee schedule.  In the Target Fee Schedule box, put in the begin date you would like the fee schedule to start. Click OK.

At this point your fee schedule will be updated with the correct amounts for 2014.

Exclusions and Rejections for Invalid Service Line Procedure G8553.

Exclusions and Rejections for Invalid Service Line Procedure G8553.
As of January 1st, 2014 the G8553 code is no longer valid and will cause rejections or exclusions. The reporting period for this Electronic Prescribing (eRX) incentive code ended on December 31st, 2013.

The G8553 code was used to report the use of an eRX in CMS’s eRX Incentive program. The program is still in effect but the code was used for reporting on the services from June to December 2013 only. In 2014 those providers who did not have an eRX in 2012, or follow the guidelines for reporting with this code in 2013 will subject to assessment. There are several other codes that where deleted as part 2014 HCPCS/CPT Procedure Codes codes updates on January 1, 2014.  The following link has these changes and the additional codes and description changes. 2014 HCPCS/CPT Update.

How do I fix the Exclusion?

Because the G8553 code was used to report the use of electronic prescribing, and isn’t used like a typical diagnosis code, simply remove the code from the visit and re-bill the claim.  When you run your claim submission wizard next the claim will go out again and will not get excluded for Invalid Service Line Procedure.

 

 

 

 


2014.1 Release Display Issues

We are receiving reports of the application not displaying properly.  If you are using IE10 or IE11 and are on our new release, you cannot be in Compatibility view.  Follow the directions below to turn off Compatibility view.

1. Click on the Tools Command Bar button. (See screenshot below)

Note: if you don’t see the Tools Command Bar,  Press and release the Alt button and the Tools Command Bar will appear.

3. To Remove a Website from the Compatibility View List in Internet Explorer

A) Select a listed webpage address that you want to turn off Compatibility View for, and click on the Remove button. (See screenshot below)

West Virginia Healthcare Payer Conversion

West Virginia Healthcare Payer Conversion

West Virginia Healthcare is converting to a Healthcare Payer Administration Solution (Health PAS) web portal. Providers must complete the required action to prevent interruption in the processing of their electronic claims and ERAs.

Action Required:  Providers must register in Health PAS by January 31, 2014 to avoid electronic claim and remittance delays.

Enrollment Changes: 

  • Payer agreements for electronic claims are not required but providers must register on the West Virginia Healthcare Payer Administration Solution (Health PAS) web portal by January 31, 2014.
  • Payer agreements for electronic remittance are required.
    • Providers already approved to receive remittance advice through RelayHealth do not need to complete a new agreement but must register on the Health PAS by January 31, 2014.
    • Providers MUST link to RelayHealth in order to receive electronic remittance through RelayHealth.  To do this, click on the  ‘Provider Associations’ bullet.  There will be a drop down box next to the 835 option.  Select McKesson Technologies dba RelayHealth from the drop down list.
    • Providers have been sent a letter from West Virginia Medicaid with instructions for registering with the new web portal.  If you have not received your letter, contact the EDI help desk at 888-483-0793; enter your NPI and choose option 6.

Additional Information for Providers:  

  • All West Virginia Medicaid providers will need to register to become users on the new site.
  •  Providers can find additional portal information by reviewing the online web portal notification at https://www.wvmmis.com/default.aspx
  •  Providers can begin to register once the access letter is received. The registration user guide is located on the web portal.

Transition Schedule:

  • January 31, 2014 – Last day West Virginia Medicaid will accept electronic claim submissions to their legacy web portal system as of 4 p.m. CT
    • Claims must be submitted to RelayHealth by 1 p.m. CT on January 31, 2014 to meet the payers cutoff time.
  • February 1, 2014 – First day electronic claims can be submitted to the new Health PAS web portal.

Deadlines extended for attesting to meaningful use

Breaking News: CMS is extending the deadline for physicians and other eligible professionals to attest to meaningful use for the Medicare EHR Incentive Program for the 2013 reporting year. .

The new deadline for physicians is 11:59 p.m. March 31, pushed back from Feb. 28. The criteria still must have been met by Dec. 31, 2013, the end of the reporting period for the incentive payments.

This extension will allow more time for providers to submit their meaningful-use data and receive an incentive payment for the 2013 program year, as well as avoid the 2015 payment adjustment,” CMS said in a news release.

Critical Security Update for IE11 Users

If you are using Internet Explorer 11 action is required

During the testing of our spring release scheduled for February 22, we discovered an issue in Microsoft Internet Explorer 11 (IE11). This issue has been found to cause sporadic crashes for some users in AdvancedMD PM and EHR. If you are running IE11 you will need to install the latest Microsoft update to avoid this issue. To run the Microsoft update, use the link: Microsoft Windows Update or for more information about the update see: Microsoft Article KB2909921. Note: this only affects IE11 users.

Additional changes might be necessary

Running IE11 in Compatibility Mode will cause display problems throughout our PM and EHR applications. The below instructions walk you through how to turn off Compatibility Mode.

How to Turn off Compatibility Mode:

1. Click on the Tools Command Bar button. (See screenshot below)

Note: if you don’t see the Tools Command Bar,  Press and hold the Alt key while pressing and releasing the T key and the Tools menu will appear

New CMS 1500 (02-12) Claim Forms

The new CMS 1500 (20-12) claim forms are available in your keys for immediate use. There are a few things to keep in mind before changing your claim forms to this new format.

1)      Contact your carriers you bill on paper claim forms, and find out if they are ready to accept the new claim form. If they are not, find out the date they anticipate to be ready.

 2)      Once you have contacted your carriers to find out when they are ready to take the new paper claim form, please follow the instructions below to update your ADP AdvancedMD system to use these forms.

To Update the New Claim Form for all Carriers:

1)      Go to: Utilities, System Defaults:

2)      Select Claims, CMS:

3)      Change the Default Paper Claim Form to: [STANDARD] CMS-1500 (02-12) as shown in the screenshot above.
4)      Click SAVE and CLOSE to save your changes.

Your ADP AdvancedMD system is now set to print the new CMS-1500 (02-12) claim form when running claims through the Claims Submission Wizzard. Please keep in mind when demanding claims, you will still need to manually select this claim form to print.

To Update the New Claim Form for only  Certain Carriers:

(*Use this option if not all your carriers are ready to process the new paper claim form*)

 1)      Follow the above instructions to update all carriers.

 2)      With a list of your carriers who are not ready to accept the new claim forms, go to Master Files, Carriers, Carriers:

3)      Pull up your first carrier who is not ready to accept the new claim form, and click on the EDI/Other button on the right hand side of the screen:

4)       On the right hand side, under Primary Form: Select {STANDARD} CMS-1500

5)       Click OK  then click  SAVE and CLOSE to save your changes.

Your ADP AdvancedMD system is now configured to print the new claim forms for  carriers who accept them, and will print the old claim form for carriers who are not quite ready to accept them.

Please keep in mind that as your carriers who were not ready to take the new claim form, become ready to accept the claim form,  you will need to follow the above instructions and change the primary form to DEFAULT  if your system default is set to the new paper claim form,  as outlined in the first set of instructions above.

iPad App Version 1.5 is live in the Apple App Store…

ADP AdvancedMD is pleased to announce our 1.5 release of the iPad application.  There are several exciting new features in this release, the key enhancement of this release is the ability to fully embrace our templates / eNotes architecture allowing seamless capture of clinical information regardless of whether they are using library content on a PC or the iPad.  Full Note temple support, bookmarks, acronym expansion, and auto-fill are all fully supported.

Microsoft Internet Explorer Bug

A new Microsoft bug was discovered on Saturday, 04/26/2014, that affects versions of Internet Explorer 6 – 11.

Microsoft confirmed Saturday it is working on a fix, but as of Monday no ETA has been announced. Microsoft typically releases security patches on the second Tuesday of each month, with the next one being on Tuesday, May 14th. It’s unknown if a fix for this will be released before May 14th or not.

Microsoft has confirmed a fix will not be issued for web browsers running on XP, after support for the operating system ended on April 8th.


What Does This Mean For AdvancedMD Users?

One of the most important things users can do is ensure automatic updates are enabled on their computers, so “important” updates get downloaded and installed automatically. We recommend users always install the latest versions of Windows, Internet Explorer, and Office (as soon as we have certified new versions).

Options to help use AdvancedMD safely until a Microsoft fix is patched.
Only one is required, not all:

  • Only use the computer to run AdvancedMD PM and/or EHR.
    Since we use no Flash in our applications, users who only use their computer to run our application(s) are not at risk.
  • Use Internet Explorer for AdvancedMD applications, and use Firefox or Chrome for everything else.
    Firefox and Chrome are not vulnerable to this bug, and Internet Explorer isn’t vulnerable if you don’t visit malicious or compromised websites that run on Adobe Flash (have Flash Applets).
  • Disable Adobe Flash.
    To Disable Adobe Flash, within an Internet Explorer window click on Tools and select “Manage add-ons”.

    Under Adobe Systems Incorporated, select “Shockwave Flash Object”, and click Disable.

Script Error in Internet Explorer 8

Update: 9:20am

A patch was released that resolved the script error.  you should no longer see the script errors.  Please keep in mind that IE8 is not a browser we support and it is likely that you will have issues with our application in the future.  it is Highly recommended that you update your browser to IE 10 or 11.

 

We are receiving multiple calls about users receiving a script error when searching for a Patient.  script error

As of January 1st we have not been compatible with IE8, official support for windows XP\Vista and IE 8 ended April 8th.

Despite the fact that we no longer ensure updates are compatible with IE8 our developers are looking in to this to determine if this is something that can be fixed.

If you are using XP or Vista it is not possible to update to IE 9 or higher.

The long term solution to this is to upgrade your PC to IE 10 or 11.

A Fix Has Been Released For Microsoft Bug

Microsoft has released a fix, which automatically be installed if automatic updates are enabled. The name of the fix will vary depending on what version of Windows, IE, and whether the computer is 64 or 32-bit. An example of what the update will look like is “Security Update for Internet Explorer 11 for Windows 7 for x64-based System (KB 2964358).

Windows 7 (all editions), & Windows 8/IE10 will be associated with KB2964358
Windows 8 or 8.1/IE11; will be associated with KB2964358 or KB2964444

To enable Automatic Updates:Please check with your IT Department before making any changes.

  • Click on Start | Control Panel

Unsupported Browser Message

unsupported browserIf you are using Internet explorer and gettign he Unsupported Browser Message when you go to our login page, there are two possible problems:

  1. You are in compatibility view.
  2. You are using Internet Explorer 7 (IE7) or lower.

If you are in compatibility mode, simply taking your browser out of compatibility view will resolve the issue.  Please reference this article if you need help getting your browser our of Compatibility View. http://support.advancedmd.com/2014/resources/configuring-ie/

If you are using IE 7 you will not be able to log in!  The version of Microsoft Windows you are using  is no longer support by Microsoft or ADP AdvancedMD.  Your only option is to upgrade your browser to IE8 or later.

It is Highly recommended that you upgrade to Internet Explorer 10 or 11.  This will ensure you have the best experience possible and the highest level of security possible.

FAQ

I’m using a supported browser but I still get the unsupported browser message how can I fix this?

If you are using IE 8 or later and are not in compatibility mode, you should be able to access our applications.   It is likely that your browser is misreporting the compatibility view settings.   A browser reset will usually resolve this issue, use the following link for instructions on how to reset Internet Explorer. http://support.microsoft.com/kb/923737 if you are still having an issue please contact support.

How do I know what version of Internet explorer I’m using?

You can use the following Microsoft website to determine which browser you are using.

It won’t let me update my browser to IE10 or IE11, my only option is IE8. What should I do?

If you do not have Windows 7 or Windows 8, 8.1 you will not be able to update your browser until you update your operating system.  Due to the complexity of upgrading your Operating system, Please refer to your local IT professional.   It is also likely that you will need to update your PC hardware to meet the minimum system requirements of  Windows 7 or higher.

Why can’t I just upgrade to IE8?

Upgrading to IE8 will allow you to access ADP AdvancedMD,  however you will be using an outdated operating system that Microsoft nor ADP AdvancedMd supports.  As we release our weekly updates to the PM and EHR applications it is entirely possible that you could lose some or all functionality.   It is HIGHLY recommended that you upgrade to IE 10 or 11.   We will block IE8 form accessing our applications in the near future.
